Crynodeb
Act as the lead professional for a defined caseload, coordinating and reviewing care plans. Undertake comprehensive assessments for individuals with complex mental health needs, including older adults. Develop psychological formulations and support other practitioners (e.g. GPs, social prescribers) to integrate these into their work. Deliver a range of evidence-based interventions, both one-to-one and in groups. Champion positive risk management and contribute to safe, empowering care planning. Work collaboratively within multidisciplinary teams (MDTs), including PCN forums and joint planning meetings. Liaise effectively with primary and secondary care professionals to ensure seamless care pathways. Support carers by assessing their needs and facilitating appropriate support or referrals. Promote mental health and wellbeing through education, information, and preventative advice.
