Purpose of the Role:
• Deliver specialist, evidence-based, patient-centred care for individuals with Parkinson’s Disease.
• Coordinate care across inpatient and outpatient settings.
• Act as a resource for patients, carers, and healthcare professionals.
• Support education, training, and service development.
Key Responsibilities:
• Clinical assessment and care planning.
• Communication with patients, carers, and multidisciplinary teams.
• Service development and policy contribution.
• Participation in research, audit, and quality improvement.
• Physical and emotional resilience in challenging environments.

To provide specialist nursing expertise, facilitating evidence-based, patient-centred care and assessment for patients with Parkinson's disease.
To co-ordinate and monitor in-patients / out-patients throughout their assessment and treatment pathway.
To work with internal and external MDT and report or escalate patient wellbeing or deterioration in health.
To promote and support patients and carers to maintain quality of life balance.
To be involved in delivery of relevant programmes of education and development for all levels of staff and to act as a resource for patients and health care personnel.
