We have an excellent opportunity for a senior mental health pharmacist to join our Home Treatment Teams based in Stockport and Oldham a. You will work in the Home Treatment Teams as an integral member of the multidisciplinary team and be responsible for delivering the clinical pharmacy service within your borough and providing Trustwide HTT leadership. There is no dispensary or out of hours commitment providing the perfect opportunity for you to develop your clinical skills and broaden your mental health knowledge and experience.
To provide, manage, organise and develop the clinical pharmacy service to the HTTs across your borough and lead developments to the clinical pharmacy service to HTTs Trustwide.
To lead, deliver, develop and evaluate specialist clinical mental health pharmacy services in the Home Treatment Teams.
To provide expert pharmaceutical advice and liaison to the Home Treatment Team, GPs, practice pharmacists, other healthcare professionals (HCPs), patients, and carers.
To act as an independent prescriber of medication where appropriate, ensuring that drug therapy is evidence based, safe, cost effective and appropriate to the individual patient.
To provide training to the Home Treatment Team, GPs and other HCPs within Primary Care around the use of psychotropic medicines.
To ensure that pharmacy services work within agreed guidelines to ensure effective medicines management
To provide cover for other pharmacists providing a service to and employed by Pennine Care NHS Foundation Trust, as requested.
You will work within a multi-disciplinary Home Treatment Team in Stockport and Oldham and act as clinical pharmacy service lead for HTTs across the Trust. You will also be supported by a lively clinical pharmacy team who are dedicated to delivering excellent services and cover community services, assessment services, specialist services and inpatient services. You will receive clinical and and management supervision and have opportunities to develop yourself and the role through education, training, team meetings and opportunities such as Quality Improvement and audit. You will be supported to undertake further qualifications as needed . The role is entirely clinical with no dispensary, out of hours or on call commitment
