Job Description



This Job Description is intended as a general overview of the main responsibilities of the job and does not include detailed instructions as to how the tasks are undertaken.



Job Title: Community Support Worker



Qualifications: Starting with the Care Certificate and work towards Health & Social Care Diploma Level 2 or higher (or willing to work towards)



Support worker: Reports to and is Accountable to the Managers



Hours: We provide care 7 days a week and 24-hour support services; this includes unsocial hours, weekends, Bank Holidays & sleep-ins.

Role Specification:

To provide person centred support and encouragement that is required to help the service user to remain as independent as possible in their lives, therefore it will be necessary to support respect and work with others, enabling the service user to live in their own home within their local community, this is likely to be achieved with a number of different people, the service user, family, friends and other professionals.



Duties include

To recognise and respect that you are a guest in the service users home at all times.

To treat service users as an adult and an individual recognising their rights to make choices and their rights to dignity, individuality, sexual preference, love, friendship and to respect and support the service user in the responsibilities that accompany these rights.

Support the service user to learn and develop skills in communication, domestic tasks, personal care, good health/wellbeing, social situations and a healthy personal financial situation.

To help the service user to participate in the community by developing a range of valued activities outside their home, promoting real choice by giving opportunities to try different things.

To assist the service user to maintain contact with family friends and other social contacts.

To ensure a high standard of care which may include assistance with personal hygiene, washing, dressing, eating, and drinking, taking to appointments, etc.

To provide practical assistance with the service user in carrying out everyday living tasks of whatever level they may need to enable them to live as comfortably safely and independently as possible, tasks may include help with cooking, cleaning, shopping etc.





To promote the health and wellbeing of the service user.



To ensure that the service users primary healthcare needs are met by accessing the appropriate facilities for medical, dental, optical and podiatry appointments, treatments etc., when required.

To ensure that all service users’ medication is handled administered recorded and stored in accordance with Step One Services Medications Policy and Procedures.

All service users are able to request Step One Services keep a spare key for emergencies, the key will be kept in a locked key cabinet; if you the support worker require a key, you must justify why you need it and sign it out and in, in accordance with Step One Services, Service User Key Safe & Spare key Policy and Procedures.

Support the service user in their financial transactions according to Step One Services Safe Handling of Service User’s Money and Property Policy and Procedures.

Ensuring the service user is involved with decision making which affects them considering their level of understanding and ability (see Step One Services Ltd.’s Mental Health Capacity Act Policy).

To ensure that the service user is involved as far as possible in determining their own routines e.g. what when and how they plan events and attempt to complete tasks.



Responsibilities



To be a valued member of our team employed to enable/support service user’s, to facilitate good quality services for the service users by contributing to creative planning and person-centred approaches, also to ensure that the service user’s lives' move in the direction they choose.

If required lead the service user’s person-centred planning process in partnership with your manager family friends and other professionals.

To continually seek to provide support in ways that utilise the support needs of the service user.

At all times communicate respectfully and appropriately to, with and about the service user.

Communicate regularly with managers and attend meetings with and on behalf of the service user.

Act as an advocate for service user’s where appropriate.

To ensure that if possible and appropriate leisure and hobby activities are pursued and if suitable undertaken



You’re Responsibilities



Support other staff members in their work.

To fill in daily the Care Vital app, weekly and monthly reports and any other paperwork that meets the needs of Step One Services Ltd.

To follow and contribute to agreed care plans, working support plans and personal centred plans with the service user.

To inform management of any changes or problems, e.g. Incidents, Accidents Appointments, Holidays, Benefits etc.

To cover absence of colleagues during periods of annual leave and sickness.

You will be required to work flexible hours.

As you are working alone, you must text Jamie Russell or Debbie at the end of your shift.

To participate in relevant opportunities for training and development, and to attend all mandatory training.

To help and maintain the service user's household.

To maintain confidentiality in respect of the service users, employees and Step One Services Ltd.

To ensure that all Step One Services Limited policies procedures are strictly observed and adhered to at all times.
