To be involved in the inpatient care of children and young people with diabetes facilitating safe discharge. To be involved with and assist families of newly diagnosed and established diabetes to develop an understanding to maintain a day-to-day management of the condition. To be involved in the community care of children and young people with diabetes and their families within a variety of settings, including clients homes, schools, and other community settings to support them through daily living To be involved in the outpatient care of children and young people with diabetes and support them through daily living and times of illness and crisis.
