As outlined above and to include NHS Priority How This Role Contributes Neighbourhood working and Integrated Care Leads the development of neighbourhood-based models, ensuring primary care, community services, and voluntary sector operate as one team around the patient. Digital-first Primary Care Identifies, tests, and embeds new technologies such as online consultations, digital triage, remote monitoring, and AI decision support tools. Prevention and Population Health Uses data to identify health inequalities and priority populations, supporting targeted preventative interventions. Personalised Care Ensures service redesign is co-produced with patients, enabling choice, shared decision-making, and self-management tools. Workforce Development and Sustainability Supports operational planning, workforce capacity monitoring, and upskilling staff in data literacy and digital tools. Service Efficiency and Sustainability Applies data intelligence to streamline processes, reduce duplication, and maximise resource utilisation across the PCN.
