Job Title: PCN Paramedic Reports To: PCN Manager Accountable To: PCN Clinical Director & The Harefield Practice Partners Location: The Harefield Practice Contract Type: Permanent Hours: Part-time (15 hours per week) Salary: £46,000 £57,000 (dependent on experience) Pro rata Other Benefits: NEST Pension Job Overview The PCN Paramedic will be a key member of our multidisciplinary team, delivering safe, effective, and patient-centred care both in practice and in the community. The role involves: Acting as a first point of contact for patients with undiagnosed conditions and minor illness/injury. Conducting same-day assessments, triage, and urgent home visits. Supporting the management of long-term conditions and anticipatory care planning. Carrying out clinical investigations (e.g., ECGs, venepuncture, specimen collection). Prescribing and reviewing medications (if qualified). Promoting self-care, health education, and preventative strategies. Contributing to service development, clinical audits, and quality improvement. Working closely with GPs, nurses, pharmacists, and other Practice staff to provide integrated care. Main Duties and Responsibilities Provide clinical assessment and diagnosis for patients presenting with undifferentiated or minor illness. Undertake urgent home visits and deliver treatment within scope of practice. Perform and interpret clinical procedures including ECGs, blood tests, and other investigations. Prescribe, review, and monitor medications safely in line with national/local guidelines (if qualified). Support the delivery of long-term condition management and preventative care. Advise patients on lifestyle and self-management strategies, signposting where appropriate. Maintain accurate and contemporaneous clinical records using EMIS. Collaborate with GPs, nurses, pharmacists, and other health professionals to ensure continuity of care. Participate in clinical audits, service evaluation, and research where relevant. Contribute to the teaching, training, and supervision of colleagues and students. Engage in continuing professional development and reflective practice. Key Skills and Attributes Strong clinical assessment and diagnostic skills. Ability to manage complex and undifferentiated presentations. Excellent communication and interpersonal skills. Effective teamwork and collaboration across a multidisciplinary environment. Good IT and digital literacy, ideally with experience using EMIS. Ability to prioritise workload, remain calm under pressure, and use sound clinical judgement. Empathy, resilience, and commitment to patient-centred care. Qualifications and Experience Essential BSc in Paramedic Science (or equivalent qualification). HCPC registration. Experience of working autonomously in clinical practice. Full UK driving licence. Desirable Independent prescribing qualification. Experience in primary care or community healthcare. Mentorship/supervision experience. Familiarity with EMIS clinical system.
