Typical day as a Reablement Worker
• Working closely with the care team to follow support plans, reporting concerns.
• Help customers regain or maintain independence to live well in their own home by carrying out every day daily living activities themselves.
• Carrying our risk assessments, support planning, goal setting and regular reviews.
• Supporting customers who have more complex needs with personal care including bathing, toileting and getting dressed.
• Supporting our customers in their own apartment who have a range of support needs including frailty and dementia.

You bring
• A level 2 or above, or equivalent in social care or being willing to work towards this
• Care Certificate or willing to complete within the first 12 weeks of employment
• If you’ve worked in a CQC care setting previously, that would be great!
• Have a positive, caring can-do attitude, and empathy to really make a difference to our customers lives.
• Previous experience of working with support plans, recording and using information concisely and effectively.
• Knowledge of local community and networks would be brilliant.
We can't sponsor you due to certificate limits. We review this regularly.
Job details
• We provide 24-hour support.
• Night shifts with some lone working, plus regular team meetings and training.
• Core shifts are Thursday, Friday and Saturday evenings.
• Occasionally our customers' needs mean your start and finish times may change. Where we ask you to be flexible, we will always try to reciprocate
• Confident using technology for plans, online learning and teamwork
• You’ll need an Enhanced DBS check done and we pay for that.
