Crynodeb
To work closely/under supervision of the team in receiving referrals and developing required support packages.Carry out a range of group and individual work under supervision of qualified team member.To follow the care plan supporting service users developing skills in activities of daily living, meaningful occupation and to become integrated into a range of community resources/facilities, ensuring service users experience a smooth transition into community opportunities.To work in partnership with others, promoting the work of the service and providing positive representation.To provide support around the physical health management of service users.Ensure confidentiality of service user information at all times.The post holder in conjunction with the multi-displinary team will support service users recovery.The post holder will maintain an individual caseload, under supervision of a qualified team member.The post holder will support service users to develop skills in activities of daily living and in engaging in community opportunities working either independently or alongside Team members. A full UK driving licence and vehicle for business use is required for this post; however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010. Please note applicants will be required to pay for their DBS check. Costs are deducted from salary following appointment. The cost of the DBS application is £26.40 (standard) or £54.40 (enhanced), this cost will be deducted from your salary over the first 2 months of employment. You are encouraged to enrol for the DBS Update Service. An annual fee of £16 per year applies.
