We are looking for Liaison Psychiatry Lead Nurse to join our close-knit multidisciplinary team in Clayponds Hospital and Rehabilitation Unit. If you are a Psychiatry Nurse interested to join a team of enthusiastic and committed professionals in delivering an integrated and rehabilitative care to patients, this role/s will be ideal for you.
The post holders will be integral part of the in-patient team. As a Registered Nurse you will be responsible for supporting your Manager in ensuring that our service users’ needs are met through the delivery of high-quality care. As a centre of excellence, you will offer a person-centred approach to their independence and well-being.
Clayponds Hospitalis a 70 bedded, medically led rehab unit that provides personalised support and care to enable people to recover from periods of acute ill-health, regain function and prepare them to return home and back to their local communities. The unit has 3 different wards; Jasmin and Magnolia being the multi-pathology wards and Rosemary is a neuro ward.
The successful candidate will join the service as a member of the multidisciplinary at Clayponds Rehabilitation Hospital.
The Lead Nurse Liaison Psychiatry (LP) at Clayponds Hospital will have a personal case load and will be responsible for the assessment, formulation and prescribing of care planning, including discharge planning within agreed protocols for patients presenting with complex mental and physical health needs. The post holder will be recognised as an expert practitioner in Liaison Psychiatry. The Lead Nurse Liaison Psychiatry post holder will provide, clinical expertise to the Clayponds Hospital patients and Multi- Disciplinary Team and will act as an autonomous practitioner in forming clinical decisions and complex management plans in the diagnosis, assessment and treatment of patients referred to the Liaison Psychiatry service.
The role will be an integral part of the Clayponds Hospital multidisciplinary team. The Lead Nurse will work within a defined scope of practice and to agreed clinical standards as defined within the London Liaison Psychiatry key competencies framework The role will be multi factorial including clinical assessment, formulation, non-medical prescribing and holistic treatments, including both mental health and physical health.
