The Role: We are seeking an enthusiastic and experienced Advanced Nurse Practitioner to join our expanding team. This is a crucial role that will involve working autonomously and as part of a multidisciplinary team to deliver comprehensive primary care services. The successful candidate will be a key player in supporting the partners to achieve our Quality and Outcomes Framework (QOF) and Bolton Quality Contract (BQC) targets. Key Responsibilities: Clinical Practice: Conducting autonomous clinics, assessing, diagnosing, treating, and managing a wide range of acute and chronic illnesses in patients of all ages. Prescribing and reviewing medications in line with current NICE guidelines and local formularies. Ordering and interpreting diagnostic tests (e.g., blood tests, X-rays). Providing health promotion and education to patients. Conducting home visits for housebound patients as required. Managing and triaging minor injuries and ailments. Working alongside our ACP, ANP and Paramedic Practitioner to provide seamless care. Quality Improvement and Audit: Actively participating in achieving QOF and BQC targets. Undertaking clinical audits to ensure best practice and compliance with national guidelines. Contributing to the development and implementation of clinical protocols and pathways. Identifying areas for service improvement and leading on relevant projects. Collaboration and Leadership: Working closely with the GP partners, ACP, ANP and Paramedic Practitioners to deliver integrated care. Providing mentorship and support to other members of the nursing and clinical team. Participating in practice meetings and contributing to the strategic development of the practice. Maintaining accurate and contemporaneous patient records using our clinical system. Person Specification: Essential: Registered General Nurse with current NMC registration. Independent Prescribing qualification. Significant experience working as an Advanced Nurse Practitioner in a primary care setting. Proven ability to work autonomously and manage a diverse caseload. Excellent communication, interpersonal, and consultation skills. Strong diagnostic and clinical decision-making skills. A commitment to continuous professional development and evidence-based practice. Full UK driving license and access to a car for home visits. Desirable: Experience with EMIS Web. (Training will be provided) Experience in conducting clinical audits. Experience in leading or contributing to service improvement projects. We Offer: A supportive and friendly working environment.
