Crynodeb
Prioritize and obtain accurate medication histories as part of the pre-admission process. Identify newly admitted patients to obtain/confirm an accurate medication history or to highlight to the ward pharmacist any existing patients who have had their prescribed medicines changed (added/stopped/dose adjustments/route change). Facilitate in a timely fashion the supply of any non-stock medication for individual patients. Attend regular discharge planning meetings to gather information on potential discharges. Raise any relevant issues with the multi-disciplinary team to ensure resolution prior to discharge Assess that any Patients Own Medications brought in from home are suitable for use and to facilitate safe use of medicine upon discharge Re-label any Patients Own Medication that has had a dose change according to local policy Counsel patients, their carers, and healthcare professional on doses, frequency, side effects, benefits of taking medicines/completing courses of medication, interactions with other medicines and using relevant resources to facilitate patient education Signpost patients to other healthcare professionals who may be able to assist with advice or compliance issues when the patient returns home Undertake the final accuracy check for dispensed medicines. Label and dispense prescriptions from outpatients, wards and other clinical areas, ensuring that standard operating procedures are followed Ensure effective stock management at the clinical areas to meet the clinical services needs and patients treatment plan Receive, document, store and dispose of Controlled Drugs with Pharmacist or authorised witnesses as per local policy Support the dispensary team where needed Adhere to relevant Finance related policies including authorisations and billing Work with relevant pharmacy Caregivers in managing medicines stock
