Job Title: Pharmacy Technician General Practice Reports to: Clinical Pharmacist / GP Prescribing Lead Location: French Weir Health Centre, Taunton Hours:Part-time (Wednesday and Thursday all day) Salary: Dependent on experience, in line with NHS Agenda for Change + NHS Pension Job Summary The post holder will work as part of the multidisciplinary team to support the safe, effective, and efficient use of medicines within French Weir Health Centre. Under the supervision of the Clinical Pharmacist(s) and GPs, you will be responsible for supporting medicines optimisation activities, improving prescribing safety, reducing waste, and enhancing patient care. You will process repeat prescriptions, reconcile medicines following hospital discharges, liaise with patients and community pharmacies, and contribute to medicines audits and quality improvement projects. The role will involve both patient-facing and administrative duties, always working within your professional scope of practice and maintaining your registration with the General Pharmaceutical Council (GPhC). Key Duties and Responsibilities Repeat Prescriptions & Medicines Administration Manage repeat prescription requests in accordance with practice protocols, ensuring accuracy and safety. Reauthorise repeat medications within agreed protocols, referring to a Clinical Pharmacist or GP when needed. Monitor prescribing patterns to support formulary compliance and cost-effective use of medicines. Identify patients requiring medication review and flag appropriately to the clinical team. Medication Reconciliation Undertake medicines reconciliation following hospital discharge, outpatient appointments, or other care transfers. Ensure any discrepancies are identified and resolved promptly in consultation with a prescriber. Update the patient record accurately to reflect any medication changes. Medicines Optimisation and Safety Assist with the delivery of medicines optimisation initiatives to improve clinical outcomes and reduce waste. Support the implementation of national and local prescribing guidelines. Participate in prescribing safety audits and quality improvement projects. Identify patients at risk of adverse drug events and escalate appropriately. Patient Support and Education Provide information to patients and carers about the safe and effective use of medicines, within your scope of practice. Offer advice on storage, disposal, and adherence strategies. Signpost patients to additional resources or services when needed. Liaison and Communication Work closely with GPs, Clinical Pharmacists, nurses, and other practice staff to ensure smooth medicines management processes. Liaise with community pharmacies, care homes, and hospital pharmacy teams to ensure continuity of care. Communicate effectively with patients, carers, and healthcare colleagues, adapting your approach to meet individual needs. Administrative and Record-Keeping Maintain accurate, up-to-date records on the clinical system in line with practice protocols and GPhC standards. Ensure confidentiality and data protection requirements are met at all times. Contribute to the maintenance of standard operating procedures for medicines management. Professional Development and Governance Maintain your GPhC registration and comply with the professional code of conduct. Participate in annual appraisal and agree a personal development plan. Undertake relevant CPD to maintain and enhance knowledge and skills. Attend and contribute to practice meetings, training sessions, and multidisciplinary case discussions. Participate in significant event analysis and reflect on practice to support continuous improvement. Other Responsibilities Work in accordance with the policies and procedures of French Weir Health Centre and the wider PCN. Contribute to the smooth running of the practice by assisting colleagues during busy periods. Undertake any other duties appropriate to the role and level of responsibility, as required by the practice. Person Specification (Summary) Essential: Registered Pharmacy Technician (GPhC). Experience in pharmacy (community, hospital, or primary care). Understanding of medicines management principles. Strong organisational skills and attention to detail. Ability to work both independently and as part of a team. Good IT and communication skills. Desirable: Experience working in general practice. Knowledge of EMIS Web or similar clinical system. Experience in medicines audits and quality improvement.
