Summary
A Vacancy at University Hospitals of Morecambe Bay NHS Foundation Trust.
We are seeking highly motivated, committed and experienced Senior Physiotherapist's to support the delivery of our service across South Cumbria.
The role will cover across South Cumbria, with bases available in Furness and Westmorland General Hospitals.
You will have the unique opportunity to influence change as we continue to develop our admission avoidance services inclusive of 2 hour urgent community response, virtual ward and rapid response.
Our speciality is admission avoidance of which therapy is a key component of the service delivery. Our aim is to support and treat acutely unwell patients in the community, with a range of escalated health and social care needs. Rapid Response's aims are preventing hospital admission and support early discharge from hospital through a Multi disciplinary approach.
The urgent community 2 hour response target and virtual wards are very much part of our service specification and builds on the visions outlined in the NHS Long Term plan. Our virtual ward beds are currently made up of Acute Medical, Respiratory and Frailty with our ultimate ambition being a generalist model.
Our team is a multidisciplinary team who provide who collectively integrate skills and share knowledge which includes performing non traditional roles and some cross boundary working. In addition to this we work closely with Primary Care, Community Teams, the Urgent Community Response Hub, District Nurses and many more.
As a Senior Band 6 Therapist you will:
Deliver high-quality urgent care
Have strong clinical decision -making skills
With your expertise and skill, strengthen the care plans delivered from your senior triage and assessments
As part of a multidisciplinary team assess and treat conditions within the patient’s own home.
Plan and implement person centre care especially focussing on frailty syndromes and Respiratory conditions, including chest physio.
Help to improve outcomes for patients as well as the patient experience both within the service and wider organisation
Work collectively to improve system working across acute, community and social care settings
Share your knowledge and skill
Utilise best practice
be able to work autonomously
Delegate work
be open and engaging for some cross boundary working ( training and competency assessment will be offered)
Demonstrate an understanding and appreciation for your own role and the different roles within the team and how they work in collaboration.
Undertake timely assessments.
Ensure assessments are of a high standard, with planning, implementation and evaluation of care plans accordingly.
We operate from three main hospitals - Furness General Hospital (FGH) in Barrow, the Royal Lancaster Infirmary (RLI), and Westmorland General Hospital (WGH) in Kendal, as well as a number of community health care premises including Millom Hospital and GP Practice, Queen Victoria Hospital in Morecambe, and Ulverston Community Health Centre.
FGH and the RLI have a range of General Hospital services, with full Emergency Departments, Critical/Coronary Care units and various Consultant-led services.
WGH provides a range of General Hospital services, together with an Urgent Treatment Centre, that can help with a range of non-life threatening conditions such as broken bones and minor illnesses.
All three main hospitals provide a range of planned care including outpatients, diagnostics, therapies, day case and inpatient surgery. In addition, a range of local outreach services and diagnostics are provided from community facilities across Morecambe Bay.
Our community services for adults are provided in people’s homes, community centres, clinics, GP Practices, community hospitals and our main hospitals. Our aim is to work with people to help them remain independent, improve their health and manage their conditions through high quality care, advice and support.
