Crynodeb

We are looking for a compassionate and skilled Registered Nurse to join our team. This role involves both completing care assessments and care plans and providing direct clinical support to service users where nursing care is required.

The successful candidate will play a key role in ensuring high standards of clinical care delivery, including wound management, catheter care, stoma care, injections, and insulin administration. This position is vital in maintaining the well-being of our service users and ensuring that care is delivered in line with CQC, NHS, and PAMMS standards.

Key Responsibilities:

✔ Clinical Assessments & Care Plans:

Conduct initial and ongoing nursing assessments for service users.

Develop, review, and update care plans to reflect changing health needs and risks.

Ensure care documentation meets CQC and PAMMS outstanding standards.


✔ Direct Nursing & Clinical Care:

Provide hands-on nursing care to service users in supported living and domiciliary settings.

Administer medication, including emergency injections when required.

Perform wound management, catheter care, stoma care, and insulin administration as needed.

Deliver clinical interventions and support service users with complex care needs.


✔ Incident & Risk Management:

Monitor, investigate, and report safeguarding concerns, medication errors, and other incidents in line with regulatory requirements.

Work closely with the Registered Manager and HR to ensure any concerns are promptly addressed.


✔ Collaboration with External Stakeholders:

Liaise with GPs, hospitals, multidisciplinary teams, and local authorities to ensure coordinated care for service users.

Attend multidisciplinary team meetings and care reviews to provide clinical input.


✔ Training & Development Support:

Assist in mentoring and supporting care staff with clinical procedures where needed.

Ensure that all nursing-related tasks are carried out in accordance with best practices.

Requirements:

✔ Qualifications & Registration:

NMC Registered Nurse (RN / RGN / RMN / RNLD) – Essential.


✔ Experience:

At least 3 years of post-qualification nursing experience.

Previous experience in domiciliary care, supported living, or community nursing is preferred.


✔ Skills & Attributes:

Strong clinical assessment and care planning skills.

Competency in wound management, catheter care, stoma care, insulin administration, and injections.

Proactive and confident in ensuring care delivery meets regulatory standards.

Strong communication skills and ability to liaise effectively with external professionals.


✔ Driving License:

A full UK driving license is required, as travel between service users’ homes is part of the role.
