Crynodeb
Ensure that safe and effective discharge planning forms a core component of care planning and delivery from the time of initial assessment, taking into consideration the contributory factors influencing a safe discharge including referrals to primary care and supportive services. The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply. Interviews to be held on 03/09/2025.
