General Actively seek to safeguard patients within the safeguarding adults guidelines andprocedures whilst promoting independence. Ensure that risks are identified and managed with the appropriate referrals to risk enablement are made. To implement and review support plans as directed by the service for patients andensuring person centered delivery of services. To work in close partnership with colleagues in the health hub, other agencies, families/carers, to achieve a coordinated service based on the support plan, within eligibility criteria. For the Vulnerable Adult Service Ensure that all records/reports, in whatever format, are completed thoroughly and accurately. To ensure that own practice is of the required standard by knowing and implementingrelevant policies, procedures, and guidance. Attend appropriate meetings and forums, relevant to the role, and actively promotethe service. Attend regularly all staff meetings and other team meetings as directed by the management. Personal/Professional Development: The post holder will participate in any training program implemented by the practice aspart of this employment, such training to include: Participation in an annual individual performance review, including taking responsibility for maintaining a record of own personal and/or professional development. Taking responsibility for own development, learning and performance anddemonstrating skills and activities to others who are undertaking similar work. Quality: The post holder will strive to maintain quality within the practice, and will: Alert other team members to issues of quality and risk. Assess own performance and take accountability for own actions, either directly orunder supervision. Contribute to the effectiveness of the team by reflecting on own and team activitiesand making suggestions on ways to improve and enhance the teams performance. Work effectively with individuals in other agencies to meet the business needs. Effectively manage own time, workload and resources. Confidentiality: While seeking treatment patients entrust us with or allow us to gather sensitive information in relation to their health and other matters. They do so in confidence and have the right to expect that staff will respect their privacy and act appropriately. In the performance of the duties outlined in this job description the post holder may have access to confidential information relating to patients and their carers, Norfolk Primary Care staff and other healthcare workers. They may also have access to information relating to the business. All information from any source is to be regarded as strictly confidential. Information relating to patients, carers, colleagues, other workers or business mayonly be divulged to authorised persons in accordance with the organisations policies and procedures relating to confidentiality and the protection of personal and sensitive data. Other: Ensure security of data and information in accordance with GDPR and the Data Protection Act, the practices policies and procedures and other legislation. Promote and maintain own and others health, safety and security, in accordance withlegislation and the practices health and safety and security policies. Undertake any other duties that are appropriate to the post.
