Summary


This is an exciting opportunity for an experienced and highly motivated pharmacy technician to join the well established homecare team at Calderdale and Huddersfield NHS Foundation Trust ( CHFT). You will work closely with the lead pharmacist to continue to develop homecare services within CHFT in this growing service. The post holder will work with key stakeholders including finance, clinical teams , internal pharmacy teams , homecare companies and commissioners in the management and development of homecare services.
• Manage and develop the homecare medicines service for CHFT in line with the Department of Health’s strategy “Homecare Medicines: Towards a vision for the future” and the Royal Pharmaceutical Society of Great Britain standards.
• Be responsible for the purchase of homecare medicines and related equipment and the accurate payment of invoices.
• Be responsible for the day to day management of homecare medicines supply dispensing services including training and assessment of technical staff to maximise efficiency and
effectiveness.
• Monitor and report service performance of homecare contracts in conjunction with clinical teams

We employ more than 6,500 staff who deliver compassionate care from our two main hospitals, Calderdale Royal Hospital and Huddersfield Royal Infirmary as well as in community sites, health centres and in patients’ homes. We also are incredibly proud to have almost 150 volunteers here at CHFT.

We provide a range of services including urgent and emergency care; medical; surgical; maternity; gynaecology; critical care; children’s and young people’s services; end of life care and outpatient and diagnostic imaging services.

We provide community health services, including sexual health services in Calderdale from Calderdale Royal and local health centres. These include Todmorden Health Centre and Broad Street Plaza.

• To coordinate and be the point of contact within the trust for homecare medicines for homecare companies, pharmacists, clinicians, purchasers, contracting and finance.
• Acts as professional secretary to all homecare related meetings and project groups.
• To ensure compliance with statuary regulations and current homecare medicines management guidance.
• Liaising with senior pharmacists, propose and coordinate the implementation of new homecare services.
• To forge links with other pharmacy homecare service providers to share best practice and contribute to homecare medicines management and policy.
• Work independently to produce and update SOP’s and other policies/documents that relate to the homecare service.
• Be responsible for the health and safety of the service, ensuring that COSHH regulations are observed.
• Liaise with procurement and regional services to ensure contracts and service level agreements are in place for homecare services.
• Communicate and resolve sourcing problems, contract changes and price issues.
• Prioritise own unpredictable workload independently and support other members of the pharmacy homecare team in the prioritisation of their workloads.
• Liaise with other pharmacy staff to ensure that the clinical requirements of homecare purchasing are met.
• Ensure accurate documentation of all homecare records with attention to detail, including the management of the homecare database (patient identifiable sensitive information)
shared between finance department and pharmacy.
• Promote the pharmacy homecare service, seeking new users and championing the service and contributing to research and pilot schemes within homecare services where appropriate.
• Identify and implement risk reduction strategies in the homecare service.
• Resolve purchasing disputes with homecare service providers.
• Ensure good, efficient and accurate patient records are maintained within the homecare medicines service.
• Investigate, develop and report on the current and future processes for patients consenting to receive homecare medicines management.
• Independently coordinate the review of existing homecare services with multidisciplinary teams. This will involve the analysis and interpretation of key performance indicators relating to the homecare service providers including reporting and resolving any issues identified.
• Produce quality and financial reports on homecare medicines services.
• Coordinate the service review meetings for homecare medicines management working closely with homecare companies, pharmacists, nurses, doctors, other healthcare staff and
patients.
• Maintain and promote high professional standards at all times.
To manage the homecare medicines which have a Patient Access Scheme ensuring that patient level data is recorded, that prescribers are aware of their responsibilities and that systems are in place to ensure finance can invoice commissioners appropriately.
• Lead the recruitment process of staff to the pharmacy homecare medicines service team by reviewing establishment needs and service changes.
• Support lead pharmacists and trust clinicians in the business management of homecare services.
• To help communicate and explain service operational procedures/changes to service users, commissioners and providers.
Investigate and manage homecare complaints and incident reporting, both internal and external in line with national standards.
