Crynodeb
Deliver a high-quality First Contact Physiotherapy (FCP) service to patients registered with SLH PCN, operating from within PCN premises. Serve as the first point of clinical contact for patients, providing expert physiotherapy assessment and decision-making regarding the most appropriate course of care, including for undifferentiated conditions. This will involve consulting directly with patientsoften without prior GP involvementto establish timely, accurate diagnoses and tailored management plans for musculoskeletal (MSK) conditions. Request, follow up on, and interpret appropriate diagnostic investigations as needed to support diagnosis and treatment planning. This includes a sound understanding of how to interpret results from investigations such as x-rays and relevant blood tests, recognising their limitations, sensitivity, and specificity. Support the implementation of self-management programmes and provide holistic, patient-centered care. This involves empowering patients to manage their conditions where appropriate, promoting healthy lifestyle choices, and enhancing their physical activity, mobility, independence, and quality of life, while aiming to reduce reliance on pharmacological treatments
