Hales Home Care is seeking experienced Care Workers / Care Assistants / Carers to join our Discharge Support Service across Gateshead.

This role involves dynamically assessing the needs of individuals and supporting their transition from a hospital stay to living as independently as possible in their own homes.

You will be required to liaise with other health professionals, and signpost service users and family members to the appropriate additional support, equipment and services to aid their ongoing recovery and long term care needs.

Areas covered:

Zone 1 – Felling, Windy Nook, Leam Lane, Deckham, Old Fold, Bensham, Teams.
Zone 2 – Wrekenton, Birtley, Harlow Green. Zone 3- Dunston, Whickham, Sunniside, Blaydon, Crawcrook, Ryton, Prudhoe.
Shifts or hours available:

You will be required to work on a rota basis between 7am to 11pm, 7 days per week.
7am to 2:30pm, 2.30pm to 11pm.
4 on 3 off shift pattern.

Requirements:

Right to work in the UK - Unfortunately, we are currently unable to accept applications from individuals requiring Tier 2 Certificate of Sponsorship.
Previous experience in delivering person-centred care.
Full UK driving licence with access to your own vehicle. (Staff will be provided with a car in each zone for the use of delivering the service.)
