Crynodeb
The post holder will be an experienced pharmacist, who acts within their professional boundaries, to develop, manage and mentor a team of pharmacists and pharmacy technicians to include regular clinical supervision, 1 to 1 support and development. They will be a non-medical prescriber and have completed the Centre for Pharmacy Postgraduate Education *CPPE* National Pathways and supervisor courses. They will work within their clinical competencies as part of a multi-disciplinary team to provide senior expertise and clinical leadership in medicines optimisation and quality improvement across the PCN. This will include structured medication reviews with direct patient contact and may include contributing to: Enhancing Health in Care Homes Management of long-term conditions Management of medicines on transfer of care Reviewing systems for safer prescribing Contributing to achievement of National and Local Contracts (e.g. IIF, QOF, CEIF, QI) Undertaking clinical audit Contributing to repeat prescription authorisations and reauthorisation Actioning acute prescription requests Addressing both the public health and social care needs of patients Minor illness *Please see full job description attached*
