Crynodeb
To secure the best possible outcomes for people who come into contact with adult social care which reflect their priorities, improve their wellbeing and enable them to live their lives as independently as possible, using a strengths-based approach which starts with identifying peoples own strengths, listening and understanding what a good life looks like to people and allows meaningful and supportive connections to be built with families, friends, and local communities. Supporting People who draw on support. Work independently with adults (and their informal carers) who have highly complex care and support needs and/or where there are court of protection issues and safeguarding adult concerns. Make, and implement, judgements and decisions with autonomy on cases with high levels of complexity, risk, uncertainty and challenge with the oversight of the manager. Plan and implement interventions designed to address needs and reduce risk. Prioritise tasks appropriately and be responsible for own work. Liaise with partner agencies to secure support and services for people drawing on support, their families and informal carers. Lead on S42 (safeguarding adult) investigations and Court of Protection processes. Manage/Chair a range of multi-agency meetings designed to monitor and progress Safeguarding Adult Protection Plans and Vulnerable Adult Risk Management. Maintain up to date records and produce written reports. Complete legal documentation and file and serve legal papers where required. Provide expertise in Social Work Practice for less experienced workers by leading by example when practising and by assisting others with complex work when help is required.
