Are you passionate about delivering seamless patient care? We’re looking for a dedicated professional to support patients throughout their inpatient journey — from admission to discharge.
Key Responsibilities:
• Support the patient’s pathway throughout their inpatient journey from admission to discharge
• Work as an effective member of a multi-disciplinary team and refer all matters outside of their scope of practice to the discharge and admissions coordinator, nurse in charge, ward manager or therapy team.
• Progress actions relating to patient’s care to meet their expected date of discharge
• Record all types of patient information accurately and have the ability to produce and maintain documents and spread sheets.

• Work as part of the multi-disciplinary team, ensuring effective communication across all departments.
• Communicate effectively with patients, carers, visitors, other staff members and other healthcare professionals at all times to ensure all are aware of pathway progression and discharge plans.
• Attend daily board rounds, MDT and other meetings to record and take actions and to expedite investigations and treatments to support pathway progression.
• Relevant onward referrals to be initiated, completed and chased to obtain final decision in regard to patient’s discharge plans.
• Update relevant data and reporting with patient information to ensure accurate timely information is communicated to the multidisciplinary team
• Maintain communication channels with the multidisciplinary team , including Allied Health Professionals, Medical Staff, Nursing staff and Social workers. Support teams to ensure action identified at the daily board rounds and beds meeting are progressing of which is of a sensitive and confidential nature
• Escalate to senior staff any barriers that may result in a delayed transfer of care
• Co-ordinate with the nursing, medical and pharmacy teams, to ensure that all medications are prescribed and dispensed prior to discharge.
• Liaise with the social services team, as directed, completing the relevant notice of discharge documentation
