The Health Board currently operate a number of different shift systems. The most common are a 12.5 hour shift, inclusive of either one 30 minute or two 30-minute unpaid breaks. These shift patterns are subject to review and the Health Board reserves the right to change working arrangements. CTM is a Health Board committed to meaningful consultation with staff and to making balanced decisions that consider all aspects of our service and workforce. Act as the patients advocate clearly presenting the patients opinions to others. Attend emergency codes when requested and provide support to clinical and operational team. To have knowledge and be aware of responsibilities under the Assessment, Care in Custody and Teamwork Process in Prison (ACCT) act within the prison environment. The post holder will ensure patient confidentiality, privacy and dignity at all times, using a range of verbal and non-verbal communication tools. The post holder will communicate factual information with patients, parents, guardians and carers who may have difficulty in understanding or communicating e.g. may be dysphasic, depressed, have auditory/visual impairment, have learning difficulties, do not have English as a first language or may be unable to accept diagnosis. Encourage patients/carers to adopt an active approach to regaining and maintaining their personal independence. This will require empathy, sensitivity, tact, persuasion and good interpersonal skills. Work and liaise with the Primary Care team as well as patients/relatives whilst adhering to all University Health Board policies and procedures. To report any change in the patients condition/environment to the Registered Nurse or other members of the multi-disciplinary team in a timely and professional manner. Contribute to the development of patient care by attending relevant team, department or other appropriate meetings and commenting on recommended changes to clinical care or service provision. Arrange and confirm appointments for annual review/follow ups/treatment by telephone, post or face to face invite. This will also include re-arranging patient appointments at short notice due to unforeseen circumstances such as cancelled clinics, which will require negotiation skills. Respond appropriately to patient queries, often dealing with anxious and distressed patients/carers face to face or by telephone. Contribute to the protection of all vulnerable adults and children and report any concerns to the Team Leader as per Safeguarding Adults and Childrens policies and procedures. Promote equality for all individuals to include promoting anti-discriminatory practice as well as supporting individual rights and choice within service delivery. Reports any complaints to the Team Leader/Registered Nurse in a timely manner. Adheres to The Code of Conduct for Health Care Support Workers in Wales".
