Job Description Purpose of role A care coordinator plays an important part within the surgery, they need to be caring, dedicated, and reliable and enjoy working with a wide range of people. They will participate fully in the running of the Practice within the scope of specified administration duties detailed below in order to provide an efficient service for the clinical staff and Patients. Being responsible to the Practice Manager and conducting oneself in a pleasant and business-like manner. Key responsibilities Manage staff annual leave and arrange cover where necessary to ensure that a minimum level of staffing is available to ensure that patient care is not impacted Manage staff sick leave and arrange cover where necessary to ensure that a minimum level of staffing is available so that patient care is not impacted. Coordinate general tasks that get generated from GPs Oversee the day-to-day operation during working hours of the reception team to ensure that staff achieve their primary responsibilities, and ensuring that the wait on the practice telephone lines is not excessive at any one time. Support and monitor staff in the use of IT; organise, oversee and evaluate IT training where appropriate. Maintain a good knowledge of the clinical systems and provide training where required to staff and patients to enhance patient care. Coordinate and support management of the daily distributions of blood results. Coordinate and support management of 111 & OOH reports including patients with complex co-morbidities. Coordinate and support management of the Breast Screening DNA follow ups. Manage or oversee income-generating contracts: Oversee QOF and generate reports on health inequalities. Oversee Minor Surgery Contract Manage Weight Management for health promotion targeting vulnerable patient cohorts. Manage 3LTC Project Oversee Learning Disability reviews Manage Hypertension project Manage Care Home Contract liaise with Care Home Managers, arrange regular review meetings and transcribe minutes. Manage UCPs Manage IIF ( See additional sheet) Manage GP Premium ( See additional sheet) Learn all areas of administration within the surgery to provide cover as and when required: AccuRx Triage Surgery Emails Medical Reports/Registrations Docman Referrals Reception General Care Coordinator role Undertake continual personal and professional development, taking an active part in reviewing and developing the role and responsibilities, and provide evidence of learning area when required. Adhere to organisational policies and procedures, including confidentiality, safeguarding, lone working, information governance, equality, diversity and inclusion training and health and safety. Establish strong working relationships with GPs and practice teams and work collaboratively with others. Demonstrate a flexible attitude and be prepared to carry out other duties as may be reasonably required from time to time within the general character of the post or the level of responsibility of the role, ensuring that work is delivered in a timely and effective manner. Identify opportunities and gaps in the service and provide feedback to continually improve the service and contribute to business planning. Contribute to the wider aims and objectives of the PCN to improve and support primary care.
