Crynodeb
You will be a member of the Neighbourhood Nursing Team and will participate in the clinicalcase management of patients who have a wide variety of complex, high-risk and supported /self-care conditions.Working as part of Neighbourhood Nursing Team, you will be responsible for your clientgroup, liaising directly with the other members of your team as well as other health careproviders to ensure the provision of a high standard of nursing care and health promotion.In order to maintain the integrity of the team and consistency of care for patients, you will worktogether to minimise the need for Bank & Agency and provide safe staffing levels. Care isprovided in a range of settings including patients homes, residential homes, nurse led clinicsand GP practices.Responsibilities include:Holistic goal setting, solution driven assessment; development, implementation and evaluation of a plan of care for patients long-term conditions, including tissue viability, continence, palliative care, personal care and supporting daily needs, promoting/facilitating patient independence.Assist in the delivery of NICE and relevant clinical policies.Providing public health and health promotion programmes to improve health and reduce inequalitiesMaintaining contemporaneous written and computerised records
