Crynodeb
Working within strategic and CNWL policy frameworks, identify assessments that are to be arranged for completion in the LAC Nurse Clinics and which are to be completed in the home environment according to the needs of the child. To liaise with LAC Administrative Support for booking RHA appointments with carers and co-ordination of the reports required. To undertake comprehensive health assessments and formulate health care plans within a holistic framework for LAC. To adopt a flexible approach to working in order to accommodate children and support young people who may be difficult to engage with. To undertake health assessments on LAC from other Local Authorities in line with good practice.
