The post holder will: Work sensitively with people, their families and carers to gather key information Coach individuals across a series of sessions to set personal goals and build the skills and confidence to then achieve them Work with the principles of self-management to actively support shared decision making, proactive engagement with social prescribing and support self-management education Promote health coaching and it's role in self-management as part of personalised care Be proactive in developing strong links with local organisations
