instruction, and patient-centred care across the designated localities.
Key Responsibilities include:
• Triaging referrals into the Falls Strength and Balance Exercise Service to ensure patients
meet the agreed eligibility criteria.
• Conducting initial face-to-face home assessments to determine suitability for either an
Otago-based home exercise programme or a Postural Stability Group Exercise Programme.
• Leading and delivering structured strength and balance home-based or group exercise
sessions in accordance with service guidelines.
• Ensuring full compliance with Trust policies, including health and safety requirements, lone
working procedures, infection prevention and control, and completion of all mandatory risk
assessments.
• Supporting the ongoing development, evaluation, and improvement of the Falls Strength and
Balance Exercise Service.
• Where appropriate, contributing to the development and assessment of competencies for
Band 3 unregistered support staff.
• Providing clinical supervision, mentorship, or task-based support to Band 3 unregistered
staff, if applicable.
• Travelling efficiently across multiple locations using own car to deliver care and fulfil service
commitments
