Summary
The post holder will be joining an expert team of registered professionals working within the wider Intermediate Care Service. The role consists of clinical triage of referrals into CPFT for discharge pathways 1 (care and therapy at home) and 2 (health interim beds in private provider care homes), and management of patients in pathway 2.
The postholder will need clinical experience of patients following orthopaedic surgery and cognitive impairment / delirium / dementia. Excellent communication skills are paramount as the postholder will liaise with colleagues in Intermediate Care, wider internal colleagues and teams, private provider care homes, care agencies and system partners such as social care and Continuing Health Care teams. The role is undertaken partially working remotely using Microsoft Teams and face to face patient assessments, reviews, and interventions.
The position covers the Cambridgeshire and Peterborough areas, with bases in Cambourne, Ely, Huntingdon or Peterborough. Main base would be Huntingdon however base could be flexible across the county as the team covers multiple locations
Please note for this role you will be required to travel independently around the county meeting strict time deadlines. You will need to hold a full UK driving licence* and have use of a vehicle. You cannot use public transport for this role as this is not a reliable form of transport and will not allow you to meet service needs. To act as a specialist registered professional in providing specialist assessment, rehabilitation and discharge planning support for the most complex group of patients with complex health needs on discharge from the acute hospital and community pathways. You will have managed flow into health interim beds and provide expert assessment, rehabilitation and discharge planning for patients on our discharge pathways in the community as part of the Intermediate Care Service across Cambridgeshire.

• To act as a link professional for complex patients discharged to the community Discharge To Assess Intermediate Care (D2A/ICT) services. This role includes clinical triage of patients referred into community services
• The identification, assessment, planning, implementing and evaluating care as a specialist practitioner. Including the completion of specialist discharge planning and specialist nursing or therapy assessments, trusted assessor assessments, equipment ordering in accordance with local and National guidelines
• Provide advice and clinical expertise to other colleagues including those from acute trusts, CPFT inpatient units, other CPFT services and nursing and residential homes to expedite discharge from patient pathways and where possible prevent admissions
• Make recommendations for health care packages for patients and assess the need for referral to other agencies and disciplines as appropriate
