Thames Reach runs projects and services that help to end homelessness in and around London. We work directly with homeless people on the streets and in hostels. We work to prevent vulnerable people from becoming homeless. We help formerly homeless people get their lives back on track. We help people find work and access training and volunteering opportunities.

About this role…
We are looking for a Support Worker to join the Greenwich Navigators team.
Thames Reach’s Supported Housing is comprised of Accommodation, Housing First, Navigator and Move on services supporting people to make real and lasting change in their lives. People may present with complex mental health, offending histories, drug and alcohol needs.
Navigator and Move On services: Provide holistic floating support to clients at different stages of their homelessness journey helping them navigate and access systems and services with the ultimate aim of securing accommodation in the community and prevent future homelessness.
We will not accept applications where Artificial Intelligence (AI) has been used to assist with completing the competency questions.
You will:
.Carry out assessments with people who referred to the service to understand their eligibility and needs
.Foster a person centered approach to support individuals with a history of rough sleeping to set and achieve goals
.Support individuals in the community to access specialist support services and secure independent accommodation, working closely with external agencies including the local authority, health services and housing providers
.Represent Thames Reach to external agencies and contribute to a collaborative work and development of the team
.Accurately record all work done in a timely manner
You will be:
.Confident working in a community setting and building trusting relationships with people who have a variety of support needs
.An organised and efficient problem solver who works well within a team and alongside other agencies
.Responsive and flexible to the needs of the team and the people we support, with the ability to manage your own caseload
.Committed to developing your knowledge and skills
.Confident in representing Thames Reach at external meetings and with outside agencies
.Good IT skills with experience using Outlook, online recording systems, Excel, and Word, using a tablet and a smart phone
