As an ARRS GP working within the Weald PCN, your day-to-day responsibilities will include: 1. Clinical Care and Patient Management Provide high-quality, evidence-based clinical care to patients across a range of settings including face-to-face, telephone, and virtual consultations. Manage acute, chronic, and complex medical conditions in line with national and local guidelines. Review and manage test results, hospital letters, and repeat prescriptions in a timely manner. Conduct medication reviews where appropriate, in collaboration with the clinical pharmacist. 2. Same Day Access and Triage Support same day/urgent care clinics, assessing and managing patients presenting with undifferentiated problems. Work with the wider clinical team (including ANPs and paramedics) to ensure patients are seen by the right clinician at the right time. 3. Continuity of Care Build trusted, long-term relationships with patients, particularly those with complex or ongoing needs. Support continuity through planned care reviews (e.g. diabetes, hypertension, frailty) and care plan updates. 4. Care Home Support and Frailty Work Participate in routine and ad hoc care home visits as part of a multidisciplinary approach to managing frail and elderly patients. Contribute to personalised care planning and end-of-life care discussions where appropriate. 5. Team Collaboration and Supervision Work closely with other ARRS staff including clinical pharmacists, social prescribers, care coordinators, and frailty paramedics. Participate in regular team meetings, case reviews, and multidisciplinary discussions. Receive clinical supervision from an experienced named GP and contribute to the learning environment of the practice. 6. Record Keeping and Administration Maintain accurate and timely patient records using the practice clinical system (e.g. EMIS). Complete referral forms, sick notes, and other administrative tasks associated with routine GP work. 7. Professional Development Engage in continuous professional development activities, with potential support to develop educator or specialist interests in the future. Contribute to the improvement of practice and PCN services, including involvement in audits, QOF, and service development where appropriate. Maintaining, Monitoring & Updating Computer Database and Management of Medical Records & Practice Audits Ensure that PCN clinical database is updated promptly and accurately with information relating to patient records, as required for clinical accuracy and for practice recording requirements under IM&T protocols, QoF, QIPP, Enhanced Services and other clinical initiatives the practice partakes in. Respond to alerts on records Appropriate use of Read/SNOMED codes (via templates where appropriate) Participating in the collection and collation of statistics towards the preparation of practice reports, performance monitoring and audits Participation in the auditing of PCN activity Participation in patient satisfaction audits Clinical Governance Process and action incoming patient and hospital correspondence Attendance at primary health care team meetings, significant event audit meetings, annual complaint audit meeting and others as deemed necessary Initiate and participate in the formulation and implementation of new protocols, policies and procedures to aid and improve administrative organisation and the provision of services to patients Ensure that the PCN complaints system is adhered to Awareness of and compliance with all relevant PCN and organisational policies/guidelines, e.g. prescribing, confidentiality, data protection, health and safety Completion of Information Governance online toolkit or attendance at group training annually and provide certificate Maintain current CPR and anaphylaxis certification and safeguarding for adults and children certificates. Professional Development Participate in an annual performance review under agreed headings Engage in appropriate educational activity to maintain clinical competence and performance Attendance and participation in educational meetings in the Practice and externally Support colleagues in their professional development Medical indemnity and registration The GP is required to effect and maintain full registration with the General Medical Council and to effect and maintain membership on an occurrence-based basis with a recognised medical defence organisation commensurate with the practitioners responsibilities.
