The Care Act Assessment Team focuses on carrying out assessment & support planning with service users throughout Herefordshire who have recently left hospital via a Discharge to Assess pathway and who may require ongoing services
Discharge to Assess experience (D2a) and/or discharge planning.
Ability to undertake fast paced and timely short-term work to meet set Discharge to Assess deadlines and progressing from Discharge to Assess.
Ability to undertake duty/welfare checks with Home First and facilitate service user’s discharges from Discharge to Assess beds.
Experience of assessment and support planning as this post will require the completion of Care Act (2014) assessments, Mental Capacity Act Assessments (2005) where appropriate, Continuing Healthcare (CHC) Checklists progressed and participation at CHC Decision Support Tool (DST) meetings as required.
Ability to travel across Herefordshire to undertake visits in person.
Working knowledge of Mosaic record system and Microsoft office/teams.
Registered with Social Work England and with relevant experience and a strong commitment to personalised, strength based, and outcome focussed practice.
