Crynodeb
Essential Skills & Experience: Experience working in healthcare, social care, or community environment. Experienced and qualified to administer vaccinations i.e. COVID-19, influenza Strong understanding of the needs of elderly and frail patients. Excellent communication and interpersonal skills. Ability to work independently and as part of a multidisciplinary team. Good IT skills and familiarity with clinical systems (training can be provided). Empathetic, patient-focused, and highly organized. Desirable: Previous experience in a Care Coordinator or similar role. Knowledge of NHS frameworks such as the Enhanced Health in Care Homes (EHCH) model or the Comprehensive Geriatric Assessment (CGA). NVQ Level 3 in Health and Social Care or equivalent.
