Crynodeb
Arranging and triaging of the electronic Holistic Needs Assessments (eHNA) during the cancer patient pathway. This will include the navigator assisting the patient with completing their eHNA where appropriate. This will ensure that patients are signposted in a timely manner to appropriate services. Ensure the appropriate management of patient information and the co-ordination of patient pathways through collaborative working with medical secretaries, MDT coordinators, clinical nurse specialists and other departments as required within the Trust To work with the CNSs to develop and embed the Living With and Beyond Cancer agenda within the skin cancer service. To assist in the development of end of treatment summaries alongside the lead CNS to ensure the timely communication of patient information to the primary care setting.
