Crynodeb
Carrying out medication reviews to support patients with complex needs, polypharmacy, long-term conditions, and acute medication queries. Supporting the practices' medicines optimisation, repeat prescribing systems, and medication safety processes. Providing evidence-based prescribing advice to clinical teams. Contributing to the management of long-term conditions such as hypertension, diabetes, heart disease, chronic kidney disease and respiratory disease. Carrying out medicines reconciliation and liaising with external organisations where needed. Participating in MHRA safety work, audits, QI projects, and the implementation of national/local prescribing guidance. Working collaboratively across the organisation, wider primary care network and external organisations to support consistent, high-quality care. Other relevant duties as required by PCN and/or practice needs.
