The Social Prescriber empowers individuals to improve their health and wellbeing through personalised, non-medical support.They will help develop the The Social Prescribing Link service, support PCN priorities, and collaborate with partners to sustain or create local initiatives. Social prescribing can help to strengthen community resilience and personal resilience while reducing health inequalities by addressing the wider determinants of health such as debt, poor housing and physical inactivity by increasing peoples active involvement with their local communities. Our PCN is evolving to become more involved with Neighbourhood priorities and assessing our population Health needs(as per our NHS contracts) Many of our patients fall into a frailty cohort, with that in mind, there would be some expectation to support our community team in the role.The addition of a Social Prescriber will support in completing home visits to proactively undertake assessment to identify holistic needs and develop high quality, personalised, anticipatory care to promote rehabilitation and prevent deterioration. There may also be work that is received from of practices to support patients one to one within the remit of a social prescriber such as supporting with social factors impacting the patients health and wellbeing.You'll be the key point of contact for referred patients, conducting holistic assessments, facilitating access to services, and following up to ensure positive outcomes. You'll also play a vital role in developing community resources and promoting social prescribing across the PCN. The Social Prescriber is to develop an understanding of services that can support these patients and carers from a holistic perspective and work with the multidisciplinary PCN team, GP practices, community and voluntary services to support the needs of the patients, families and carers. The post holder will work with individuals from various cultural and social backgrounds. The ability to perform effectively in a diverse and occasionally demanding environment is required.
