CLINICAL To lead on identifying patients who may meet the criteria for NHS Continuing Health Care fast track funding. To be actively involved in discharge planning and co-ordinate discharge arrangements. To lead on the nursing contribution as part of the Multidisciplinary team in facilitating effective fast track discharge To attend and take the lead if necessary in pre discharge MDT meetings. To identify patients who will require nursing intervention on discharge and liaise with both District nursing teams or palliative 24hour care settings. To ensure effective safe transfer of patients from hospital to community. To undertake assessments in relation to fast track discharge planning To work collaboratively and cooperatively with all members of the multidisciplinary team To attend any relevant meetings that may be appropriate to the fast track discharge team. MANAGEMENT To liaise with the District Nurse Coordinator, with regards to any ongoing issues or concerns that may arise, thus ensuring the smooth running of the community discharge team. To support any junior staff members with any concerns they may have. To undertake any PDRs and 1:1s for junior staff as appropriate. To undertake the management of the community discharge team in the absence of the District nurse Coordinator. To take an active role in any audit activity in relation to the community discharge team. To ensure that any appropriate information is made available to the District Nurse Coordinator. EDUCATION To support the development of teaching packs and provide educational training as required to the ward staff on issues surrounding palliative fast track patients. To participate in staff development and the PDR appraisal programme. To be responsible for the health and safety of self and others, by adhering to the principles of all Health and Safety Policies and Procedures. COMMUNICATION To use effective interpersonal skills when communicating with patients, ensuring sensitivity when dealing with emotive situations. To maintain self- awareness and use skills to interpret verbal and written information from a variety of sources. To effectively be able to communicate with relevant multi-agencies to ensure effective fast track discharge planning. PROFESSIONAL 1. To maintain own Personal and professional development and clinical competencies via on-going Continuing Professional Development (CPD) 2. To maintain a personal professional portfolio by collecting evidence of personal development through reflective practice. 3. To ensure that all Statutory and mandatory training is attended in accordance with Trust policies. 4. To work within Nursing and Midwifery Council (NMC) guidelines, and support the Locality Manager in ensuring these are adhered to. 5. To encourage others to also abide by the NMC Code of Professional Conduct. 6. To bring any professional practice issues to the attention of the District nurse coordinator. 7. To maintain accurate, comprehensive and up-to-date documentation in line with the NMC Guidelines for Records and Record-keeping 8. To share best practice and undertake all activity within the framework of Clinical Governance 9. To participate in the staff appraisal scheme 10. To inform District nurse coordinator of any changes that may be required to be made to policy/procedure due to change in practice and be actively involved in any working parties. 11. To support the District Nurse Coordinator in developing the service by pro-actively networking with other professionals and agencies to enhance service delivery. 12. To accept responsibility for own professional practice as a reflective accountable practitioner.
