Summary
Demonstrate clinical excellence and maintain consistently high standards of clinical practice in palliative care. Act as a role model providing clinical leadership for colleagues. Advise and support on patients with complex needs and their families at home, maintaining an approach that incorporates physical, psychological, social and spiritual aspects of care, in accordance with the organisation mission and values. Manage a caseload of complex patients promoting the highest possible standards of individualised care, symptom management and bereavement support through a process of assessment, planning, coordination and evaluation. Effectively allocate and support team members to implement the care plan, monitoring and evaluating the care provided, delivering practical nursing care when appropriate. Monitor and prioritise team referrals.
