We’re looking for two dedicated Social Workers to join our dynamic and compassionate Older Adult Community Mental Health Team in Bath and North-East Somerset (BaNES). About the Team Our team is part of a recently transformed service aligned with the Community Mental Health Framework. We support older adults experiencing mental ill health and complex neurodegenerative conditions, helping them live safely and independently. We are a truly integrated, multi-disciplinary team made up of professionals from both the local authority and the Avon and Wiltshire Mental Health Partnership (AWP). You’ll work alongside nurses, psychologists, medics, allied health professionals, and support workers in a collaborative, co-located environment. About the Role We are recruiting for two Social Worker II positions: One based in Midsomer Norton, covering rural North -East Somerset One based in Bath, covering the city area Flexibility to work across both locations and the wider BaNES area is essential. As a Social Worker II, you will: Manage a varied caseload, including complex and higher-risk cases Undertake assessments of need and risk, and develop strengths-based, outcome-focused care plans Lead safeguarding enquiries and contribute to multi-agency decision-making Work collaboratively across health and social care to deliver high-quality, person-centred support We are also open to applications from newly qualified social workers with relevant experience and a passion for working with older adults.
