About the role:
To assess and manage the complex needs of older people and their carers, ensuring their health, wellbeing, and social inclusion. The role involves safeguarding, promoting independence, and working collaboratively with other professionals and agencies.
About you:
Strong communication and IT skills.
Ability to manage caseloads and work under pressure.
Conflict resolution and crisis management.
Commitment to equality, confidentiality, and professional standards.
What you will do:
Conduct holistic assessments and care planning.
Manage a small but complex caseload.
Undertake safeguarding investigations and promote social inclusion.
Collaborate with Reablement and other services.
Carry out Mental Capacity Act assessments and Best Interests decisions.
Prepare court and specialist reports.
Maintain accurate records and documentation.
Promote equality, diversity, and positive risk-taking.
Mentor newly qualified practitioners.
Engage in supervision and continuous professional development.
Contribute to service development and team learning.
This position has a requirement for an Enhanced DBS Check
