Qualified Social Worker – Hospital Discharge (D2A Pathway) – Slough
Overview
We are seeking a highly experienced and self-motivated Qualified Social Worker to join a dynamic and fast-paced Hospital Discharge (Discharge to Assess - D2A) Team based in Slough. This is a frontline role requiring advanced knowledge, autonomy, and confidence in managing complex discharges and transition planning.
Role Details
Location: Slough – Hospital Pathway D2A Team
Position: Locum Qualified Social Worker
Duration: Ongoing contract
Hours: Full-time (Mon–Fri)
Pay Rate: Competitive, dependent on experience
Start: ASAP
Key Responsibilities
Lead on complex hospital discharges within the Discharge to Assess (D2A) framework.
Progress cases independently and within statutory timeframes.
Coordinate and implement timely long-term care and support packages.
Conduct assessments and manage funding pathways including Continuing Healthcare (CHC).
Navigate family dynamics and multi-agency working with professionalism and empathy.
Ensure full compliance with the Care Act 2014, Mental Capacity Act, and safeguarding procedures.
Attend CHC and safeguarding meetings as required.
Maintain up-to-date and accurate case records using Liquidlogic (LAS).
About You
Minimum 5 years post-qualified experience in Adult Social Care.
Proven background in hospital discharge, D2A pathways, or similar fast-paced settings.
Confident decision-maker able to work autonomously without constant guidance.
Strong understanding of Care Act, CHC processes, safeguarding, and risk management.
Excellent communication, planning, and organisational skills.
Calm and confident under pressure, capable of managing complex discharges effectively.
Must be able to travel within the local area for assessments and meetings.
Experience using Liquidlogic (LAS) is advantageous.
Essential Requirements
Social Work qualification (BA/MA/DipSW).
Current SWE registration.
Enhanced DBS on the Update Service or willing to apply.
Right to work in the UK.
