CLINICAL To undertake the assessment of patients and clients with complex care needsincluding individuals undiagnosed conditions. To develop, implement and evaluate specialised programmes of care to meetthese needs. To communicate effectively with patient/clients and their significant others,taking into account their cultural background and cognitive functioning. To guide, advise and direct other staff in the assessment, planning,implementation and evaluation of specialised patient and client care. To safely administer SACT to in-patients and out-patients. Identify, assess and take action in SACT intra-treatment emergencies, takingappropriate action to minimise risk to patients (including: extravasation,anaphylaxis/allergic reactions, neutropenic sepsis). To develop and maintain own clinical competency, knowledge and skills incannulation and safe administration techniques required for SACT andsupportive therapy treatments. To be competent in cannulation, phlebotomy and managing central venousaccess devices e.g. PICC, Groshong, Hickman and Portacath To initiate risk assessment processes to determine risks to the health andwell-being of patients with complex care needs, taking relevant action tominimise these risks. To provide specialist advice to staff in managing emergency and other acutesituations, ensuring that the patients/clients needs are met and thatsignificant others are supported. To utilise information and data from a range of sources to identifypatient/client problems and guide decision-making processes. Within field of specialist expertise, and organisational policy, request specificclinical tests to establish a diagnosis. To promote service user involvement in the planning, delivery and evaluationof care, respecting their wishes, beliefs and dignity. To independently liaise and communicate effectively with all members of themultidisciplinary team and other agencies involved in the care of thepatient/client. Maintain accurate clinical records of care to contemporary standards developing and utilising electronic methods as appropriate. TRAINING AND EDUCATION To take a lead in the promotion of the health and well-being of clients/patients and their significant others, ensuring that health promotion isincorporated in the planning and delivery of specialist care. To act as a supervisor to staff involved in implementing specialisedprogrammes of care. To act as a practice supervisor to students and other learners. To promote an environment that is conducive to quality learning andassessment. Develop programmes of learning to meet the educational needs of all staffworking within the care setting. Maintain own continuing professional development needs. The post holdershould hold a diploma and be willing to work towards achieving graduatestatus within specialist field. Develop and maintain competence in the additional specialist skills related tothe needs of the clients and patients. To undertake in-house annual mandatory training in Manual Handling, BasicLife Support, Fire Safety and AED training and other courses essential to role including e learning. LEADERSHIP/MANAGERIAL To ensure that clear systems of communication are developed between thepost-holder and relevant staff working within and outside the organisation. To ensure staff are aware of organisational objectives, policies andprocedures related to the specialist area and the implications this may havefor their practice. As appropriate, support staff with performance issues developing actionplans to address developmental needs. To act as a professional role model for junior staff, promoting high standardsof practice and strong professional values. To co-ordinate nurse-led and multi-agency activities within field of specialistpractice. To manage conflict between individuals, including staff, patients/clients andtheir significant others. To act as an advocate for patients/clients, significantand more junior staff members. Comply with the requirements of the Data Protection (Bailiwick of Guernsey)Law 2018 concerning improper disclosure, misuse or breach of confidentialityin respect of information held on computer systems or otherwise; makedisclosures only with patients consent or within HSC policies on ConfidentialInformation (G122) and Sharing of Person Identifiable Information (G308)
