Develop and implement targeted management plans for prevalent long-term conditions within these communities, notably hypertension and lipid management in the Indo-Pakistani population (considering genetic predispositions) and other relevant conditions prevalent within the Roma community. Proactively address taboo health areas such as smoking and alcohol use within both communities, employing culturally sensitive approaches to identify hidden habits and provide appropriate support, particularly for women within the Indo-Pakistani community and considering the specific cultural contexts within the Roma community. Identify and support patients experiencing domestic abuse, demonstrating a deep understanding of the cultural barriers to disclosure within both Indo-Pakistani and Roma communities and providing access to culturally appropriate resources and safeguarding pathways. Conduct regular, proactive community outreach specifically tailored to building trust and rapport within both Indo-Pakistani and Roma communities, engaging with mosques, temples, Gurdwaras, Roma community groups, and other relevant projects to promote health and wellbeing. Provide sensitive and non-judgmental care to individuals navigating issues related to sexuality and sex before marriage, with a nuanced understanding of the diverse religious and cultural contexts within the Indo-Pakistani and Roma communities, ensuring access to appropriate advice and services, including contraception where needed. Maintain accurate and up-to-date patient records, paying particular attention to recording relevant cultural and social factors impacting health. Participate in practice meetings, clinical governance activities, and continuing professional development, with a focus on learning about the specific health needs and cultural contexts of the Indo-Pakistani and Roma communities. Contribute to and provide written reports for an NHS Trust environment and beyond to report on the progress of the role and its outcomes. Work collaboratively with other healthcare professionals, community health workers, and interpreters to ensure culturally appropriate care. Contribute to the development of culturally specific health promotion materials and initiatives targeted at both the Indo-Pakistani and Roma communities. Adhere to all relevant NHS guidelines, practice policies, and ethical standards, with a strong commitment to reducing health disparities.
