We are seeking to recruit a highly motivated Community Learning Disability Nurse. This role will suit someone who enjoys variety, challenge and utilising their wide range of skills. Our work is about as diverse as it gets covering all aspects of physical and mental health and offers the post-holder a multitude of opportunities to develop their career.
You'll be part of a wide and supportive MDT offering the opportunity to work collaboratively with other health professionals in order to meet the specialist health needs of people with a learning disability living in North and West Oxfordshire.
Part of your responsibilities will include managing a caseload of patients and being accountable for assessing (including comprehensive nursing assessment), care planning and implementing nursing interventions (including variety of patient care plans such as Positive Behaviour Support, epilepsy, which are evidence-based).
You will also co-ordinate care for adults who have a learning disability utilising the Complex Health review processes or a case co-ordination approach, as well as contributing to effective multidisciplinary working within the Learning Disability Team and help to build strong working partnerships with other health and social care agencies.
Main duties of the job- Manager a caseload of patients who have a learning disability under indirect supervision and be accountable for assessing (including comprehensive nursing assessment), care planning and implementing nursing interventions (including variety of patient care plans such as Positive Behaviour Support, epilepsy and including the views of the patient and carers in these care plans), which are evidence-based.
- Co-ordinate care for adults who have a learning disability utilising the Care Programme Approach or case co-ordination approach.
- Contribute to effective multidisciplinary working within the Learning Disability Team and build effective liaison with other health and social care agencies.
- Undertake physical observations as follows: blood pressure, temperature, respirations, blood sugars, urinalysis, weight and height when required
- To work in partnership with others to identify and assess the nature, location and seriousness of the particular risks. To ensure that all clinical and risk assessments are completed and reviewed in line with Trust policies. To work with other members of the multi-agency team to plan, monitor and review the effectiveness of the risk management plan.

The North Oxfordshire Learning Disability Service is a specialist service within the NHS in Oxfordshire supporting individuals with a diagnosis of a learning disability with their identified health needs which cannot be met by mainstream services with reasonable adjustments.
We are a multidisciplinary team including nurses, occupational therapists, physiotherapists, psychologists, speech and language therapists and psychiatrists.
We believe in a person-centred approach, helping our service users to advocate for their health needs in a safe and professional environment. It is the duty of care for all healthcare professionals in the Learning Disability Service to advocate and empower individuals with a learning disability to make choices for themselves (where possible) and to be a part of their community.
Out team of nurses in the north of Oxfordshire is well resourced with a Lead Nurse, 5 Community Nurses, Nurse Cadet and Nurse Associate trainees.
The team work Monday-Friday 9am-5pm. We have an office base in Banbury but we also work flexibly from home and from other Trust premises to meet the needs of our patients.
As this role covers a number of bases you will need to be flexible in your ability travel and applicants must have a full valid driving license and access to a vehicle for work. Adjustments can be considered in line with the Equality Act 2010 where required.
As a member of the nursing team, the Community Learning Disability Nurse will work as part of the multi-disciplinary team to assess, plan, implement and evaluate the specialist health care needs of adults who have a learning disability, embedded within the framework of:
- MDT principles of working
- The holistic model of care (Four Tier model)
- 7 areas of clinical practice
Other tasks will include:
- Managing a caseload of patients who have a learning disability under indirect supervision and be accountable for assessing (including comprehensive nursing assessment), care planning and implementing nursing interventions (including variety of patient care plans such as Positive Behaviour Support, health action planning, epilepsy and including the views of the patient and carers in these care plans), which are evidence-based.
- Co-ordinating care for adults who have a learning disability utilising the Complex health review or case co-ordination approach.
- Contributing to effective multidisciplinary working within the Learning Disability Team and build effective liaison with other health and social care agencies.
- Ensuring that all clinical and risk assessments are completed and reviewed in line with Trust policies. To work with other members of the multi-agency team to plan, monitor and review the effectiveness of the risk management plan.
- Recognising potential crisis situations and acting responsively and responsibly, to resolve the situation seeking assistance, if required.
- Providing clinical and management supervision to staff in the team, such as Nurse Associate Trainees, Nurse Associates, Assistant Practitioners, or student nurses.
- Acting as a role model providing specialist clinical knowledge to colleagues through MDT meetings, for referred individuals and carers/relatives when required.
- Registered Learning Disabilities Nurse or working towards
- Computer skills including use of NHS clinical systems
- Mentor/practice assessor course completed
- Evidence of team working
- Awareness of personal and professional boundaries
- Honest and trustworthy
- Awareness of the importance of user involvement in their care plan
- Experience of MDT working
- Communicate effectively with people who have difficulty understanding
- Organisational skills
- Ability to work under supervision.
- Knowledge of the application of the NMC Code of Conduct
