Crynodeb
The post holder will be responsible for the management and coordination of patients requiring the Virtual Ward. The Virtual Ward Specialist Nurse will oversee/undertake the ongoing assessment of patient needs, the development, implementation and evaluation of programmes of individualised care to patients as determined at Virtual Ward Multi- disciplinary Team meetings. They will deputise as required in the absence of the team leader to ensure continued efficient and effective service delivery. They will work as part of the integrated community health care team, working within and across professional and organisational boundaries covering the 24 hour period of service delivery. They will refer to and liaise with other services within the Central Cheshire Integrated Care Partnership, when patient need requires it. They will assist and support the Virtual Ward team leader in providing clinical leadership to the Virtual Ward team and to assist in leading the team in the provision of high quality, evidence based programmes of care.
