Angus Health and Social Care Partnership is offering an exciting opportunity to join our integrated Under 65s Community Mental Health Service as an Assistant Practitioner.
Based across both North and South Angus, this full-time role plays a vital part in delivering high-quality, person-centred care. You'll be at the heart of a multidisciplinary team that includes mental health nurses, social workers, occupational therapists, psychologists, support workers, and psychiatrists—working together to provide holistic support for individuals and their carers.
Your primary focus will be to assess and support carers, ensuring their voices are heard and their needs are met. You'll champion the Triangle of Care approach, encouraging meaningful collaboration between service users, carers, and professionals.
In this role, you will:
• Provide tailored support, advice, and guidance to carers
• Promote carer involvement in care planning
• Work closely with a range of professionals to deliver integrated, compassionate care
• Contribute to training initiatives and continuous service improvement
• Contribute to Angus Health and Social Care Partnership delivering its legal responsibilities around Carer Support and Self-Directed Support
We’re looking for someone who brings strong interpersonal and communication skills, with the ability to work both independently and collaboratively across agencies. You’ll need to demonstrate a commitment to community care principles and social work values as well as an integrated approach to care.
This is a challenging, exciting, and rewarding role which offers the right candidate the opportunity to further develop a triangle of care approach within the community mental health team, improving outcomes for carers.
You will joining a supportive team and will be provided with a period of induction and ongoing support from the Social Work Team Manager and Senior Practitioners. You will have access to flexible working and elements of home working is available.
