A Vacancy at Sussex Community NHS Foundation Trust.
This is an exciting opportunity for an experienced SCPHN to work in the Healthy Futures Team (HFT) in Brighton and Hove based in a busy Family Hub in the city centre. The Healthy Futures Team is a commissioned specialist Public Health Visiting and School nursing service that utilizes early intervention models to improve Health outcomes and reduce inequalities for families facing disadvantages in the city .It is a value -led service that employs a person centred approach building therapeutic relationships working in a relationship based and trauma informed way.
The team employ a a whole system approach to their work to ensure the best outcomes are achieved for the families and children
The team have other specialist roles working with Teenage Parents , SEND, Emergency housing and woman's refugee . The postholder will be part of an excellent , well established skill mix team and have access to regular supervision and professional development opportunities .
In addition, the postholder will also work with vulnerable transient families within Brighton and Hove, including those in emergency accommodation, asylum seekers, refugees with complex needs, undocumented migrants, Gypsies and Travellers and those needing refuge from domestic violence.
· The Post holder provides supervises and supports the delivery of an effective programme of childhood developmental surveillance and health promotion activities both within the organisation and with community partners.
· Ensures effective liaison and communication with other professional groups within the Trust and outside agencies, acts as a health information source and works in partnership in the interests of clients.
· Identifies community health needs in partnership with health and other agencies, in order to jointly develop and deliver appropriate services. Takes a significant role, in partnership with others, in the development of public health initiatives for the Traveller, BME and vulnerable transient families communities, including the implementation, monitoring and evaluation of effective interventions. Promotes, offers and administers immunisation, following the routine immunisation schedule, both at on-site Traveller locations and in clinic sessions.

T The Post holder, working within Sussex Community Foundation Trust (SCFT) acts as a specialist resource for other Health Visitors and BHCFS staff with regard to the heath and cultural needs of all BME families, providing expert knowledge advice and information for others developing and delivering training sessions and works with others on service developments.
The post holder has the responsibility for the assessment of care needs, the development, implementation and evaluation of programmes of care and public health developments, working with traveller families on sites and whilst being housed.
d The post holder has the continuing responsibility for the management of a defined community caseload, team members and resources. There will be liaison with other agencies, supervision, deployment and teaching of other staff and/or students.
Leads designated team members by supporting continuing development and offering supervision, according to the Trust’s supervision strategy.
Actively supervises, guides, mentors and supports the integrated team members to ensure that the delegated aspects of the care plan are delivered following evidence based practice and up to date guidelines.
Actively engages in the recruitment and selection of team members with support as appropriate from the Service Manager and manages the induction and orientation programmes for new staff.
