Crynodeb
To carry out assessments and therapy activities with patients with non-complex needs. This will include gathering of history and physical examination in order to plan and deliver an effective therapy intervention following Trust clinical standards, guidelines and SOPS Prepare patients for treatment ensuring a safe environment. To assist patients with independent prescribed practice provided by registered therapists. To work alongside therapists/other assistants to run patient activity groups as required. To be required to work alone in line with relevant policies. To lead on audits as requested by the team lead, including audits to review compliance with documentation standards Modify and update care and therapy goal plans to reflect patient needs and participate in weekly goal setting meetings. Make referrals to other agencies e.g Outpatient Therapies or Discharge to assess Assist in ordering relevant equipment to promote patients independence. Monitor stock and keep up-to-date with documentation i.e. filing and photocopying.
