Crynodeb
Please refer to the attached Job Description for a full list of duties and responsibilities. You will be working in primary care settings working with specific PCN's, Integrated Care Teams and with Community Mental Health Teams /other specialist teams. The post requires working autonomously and as part of a team. Resilience and adaptability in approach are expected as well as the ability to offer and receive support from colleagues within the team. You will be expected to spend time developing good working relationships with colleagues across Primary and secondary care, VCSFE and local networks to facilitate collaborative integrated working. The Step Forward Mental Health Team are also responsible for the triaging and routine assessments of all local referrals into mental health services.
